This is a PPO plan.
You have to meet your yearly deductible before they pay anything.
In-network they pay 80% after the deductible. The in-network deductible was $2,800 as of 10/17/2022)
Out-of-network they pay 50% after the deductible. The out-of-network deductible was ($5,600 as of 10/17/2022)
This benefit is good for two devices every year.

Currently Pure Hearing is not in-network with Anthem BCBS - Northrop Grumman. We are dilegently working to become a provider with them. At this time we recommend visiting Anthem’s website and selecting a local, in-network provider.

As long as you shop with an in-network provider your out of pocket cost should be very low. Look at some of our prices and then deduct 80%. That is what you should be paying out-of-pocket.
